Students, faculty and staff can submit U.S. passport application documents on campus, whether or not you plan to study abroad. NY Passport Agency personnel will be available to review and accept your completed applications.

Please bring the following:

• completed but unsigned DS-11 form
• passport photo
• check (leave amount blank)
• proof of citizenship (ie. expiring passport, birth certificate or certificate of citizenship)
